Family Care Network, Inc. (FCNI) is proud to announce the recent launch of its Enhanced Care Management (ECM) Program, an important expansion of services designed to provide comprehensive, coordinated support for individuals with complex health and social needs. Through this program, FCNI strengthens its ability to deliver whole-person care that addresses the many factors that influence long-term health and wellbeing.
Enhanced Care Management is a Medi-Cal benefit that provides intensive care coordination for individuals who face significant barriers to health and long-term stability. The program supports a wide range of individuals, including children and youth involved in the child welfare system, youth and adults transitioning from incarceration, individuals and families experiencing homelessness or housing instability, and those with complex behavioral health or medical needs.
At the heart of the ECM program is a commitment to building a strong network of care around each participant. FCNI care managers work closely with individuals, families, and key partners including healthcare providers, behavioral health professionals, County agencies, and community organizations to develop and implement coordinated care plans tailored to each person’s unique goals, strengths, and needs. This collaborative approach ensures that individuals receive the right support at the right time.
“Enhanced Care Management allows us to deepen the work we already do by ensuring individuals and families have a dedicated partner helping them navigate the complex systems that impact health outcomes,” said FCNI’s ECM Program Manager. “By bringing services together and focusing on the whole person, we can remove barriers and help create stable pathways toward health, housing, education, employment, and long-term success.”
Key services provided through FCNI’s ECM Program include comprehensive care coordination across medical, behavioral health, and social service systems; individualized care planning based on an assessment of each participant’s strengths and goals; support accessing housing, healthcare, and community resources; advocacy and system navigation; and ongoing monitoring to ensure care plans remain responsive and effective.
FCNI’s ECM services are grounded in the organization’s long-standing commitment to person-centered care that prioritizes dignity and lasting opportunity for the individuals and families we serve. Care managers use relationship-centered practices and partner with families and natural supports to ensure services are respectful, empowering, and sustainable.
Since its founding in 1987, Family Care Network has worked to strengthen our community by providing trusted and compassionate services to children, youth, and families. By connecting individuals to the right services at the right time and aligning systems to better serve those with complex needs, FCNI’s Enhanced Care Management Program strengthens the safety net for some of our community’s most vulnerable residents. This expanded service reflects FCNI’s ongoing commitment to building healthier individuals, stronger families, and more resilient communities.
